If an injury or incident has just happened, report it first. Reporting an incident and filing an insurance claim are two separate steps — complete Incident Reporting, then use the HSR Claim Form to process the claim.

INSURANCE & ACCIDENT INFORMATION

Documents for each of the following are in the files below.

Description of Coverage — council specific description of coverage and outline of who is eligible for coverage.
BSA Accident and Sickness Plan — general description of coverage for those eligible.
HSR Claim Form — claim form to be filled out in order to process an insurance claim.

ANNUAL HEALTH & MEDICAL RECORD

Every Scouting participant needs a current Annual Health and Medical Record on file with their unit. Parts A and B are required for all participants; Part C requires a physician's exam and is required for events longer than 72 hours.
